- Where does occupational therapy fit in the organizational chart for your facility? I am currently at Orlando Regional Medical Center, Central Florida's only level one trauma center. This is a large facility, specializing in trauma, cardiovascular services, orthopedics, neurosciences, and internal medicine.  The rehabilitation department is staffed accordingly, with about 7 PTs, 5 OTs, and 2 SLPs on the floor during the week. Each specialty has a lead therapist and each discipline has a supervisor.  Under these supervisors and lead therapists are the full time therapists, PRN therapists, COTAs/PTAs, and rehab techs. Therapists work closely with physicians, nurses, respiratory care, dietitians, cases managers and other healthcare professionals in order to provide patients with the best care possible. 

- Of the following categories, which best describes the type setting of this fieldwork experience?  Acute; OTs at this facility are assigned patients depending on which unit they have been admitted to.  My FWS is the burn specialist and primarily works in the Burn/Trauma ICU and Burn/Trauma Step Down units. 

- List three fieldwork objectives/ goals you and your fieldwork supervisor (FWS) have established. Choose one of those objectives and share how you believe this goal will be met. 

  •  Independently manage full caseload in the last 2 weeks.
  •  Present in-service within weeks 7-12 of fieldwork.
  •  Visit other Orlando Health sites to gain better understanding of continuum of care within weeks 7-12 of fieldwork.
In order to i ndependently manage a full caseload in the last 2 weeks, it is important that I m anage approximately 1/4 of my FWS's caseload by the end of week 3.  In order to do this, I must be thorough in my chart reviews, consultations with other disciplines, and interviews with clients.  I also must learn to manage lines and tubes, as most of our patients are in the ICU and step down units and are attached to various lines.  I must refine my documentation and clinical skills, taking the opportunity to ask questions when necessary.  I have already met this goal, independently seeing 2-3 out of my FWS's 8-12 patients daily.

This is such an awesome site Jessica and you are going to learn so much with this supervisor.  Of the goals you have stated, which do you think will be the most challenging?  Of what you have mentioned how do you think USA could have better prepared you to meet these challenges?

Ms. LeSage, The most challenging part so far has been learning to manage all of the lines attached to our more involved patients. It often takes more time to manage these lines than it does to perform the transfers themselves. I am sure that with a couple more weeks of practice I will feel more comfortable managing lines and transferring these patients on my own. Unfortunately, I feel like this is something that needs to be learned firsthand, and am not sure of how USA could better implement this into the curriculum. Jessica
