Describe two issues that limit your ability to use occupation-based interventions and discuss strategies that could be used to minimize these barriers.

Two issues that limit my ability to use occupation-based interventions is that reimbursement is dependent on demonstrating improvement in FIM scores, and most patient are here only 2-3 weeks. Treatment is based on improving scores, therefore most treatment is limited to what area we are scoring as OT. Another barrier is most patients are at a level that requires an approach to treatment at a purposeful activity level of treatment. How I over come these barriers is to try to make everything functional no matter what the circumstance.  
 
For the Demographics of Treatment Sessions specify the Percent for each of the following statements:

Occupation-based activities i.e., play, shopping, ADL, IADL, work, school activities, etc. (within client’s own context with his or her goals) ___20____%

The FIM scores should represent OT interventions, should they not?  The short stay become a chllenge to addressing occupational based services.  Think outside the box and ask yourself,what does this person need to have before they leave here.  Then make a plan to address these areas.  Sound like the setting is really driving the type of interventions.

Yes, it is addressing OT intervention but on a limited scale, in my opinion. We are mainly just using our skills to improve the bathing and dressing categories of ADL intervention.  I have been and will continue to think outside the box for ADL intervention at an occupational based level. I just feel there is more to OT treatment at an occupational based level than just bathing and dressing...
