USAHS is committed to providing an academic environment that is respectful of the religious beliefs of its students. As part of this commitment, USAHS will make good faith efforts to provide religious accommodations to students whose sincerely held religious beliefs conflict with a University policy, procedure, or other academic requirements unless such an accommodation would create an undue hardship.

Verification

To request an accommodation, a student must provide verification from an appropriate religious official. Verification should include:

1.) The name, title and religious order of the religious official.

2.) The religious holiday, holy day or custom for which the accommodation is being requested.

3.) The recommended accommodation for the academic environment.

 

Accommodation Process

STEP 1. Provide religious verification and a completed Religious Accommodation Request (see below) via email at accommodations@usa.edu

STEP 2. Student Accessibility and Welfare will send confirmation of receipt of the verification and request form to the student's USAHS email. Student Accessibility and Welfare will review the request within 10 business days of receipt.

STEP 3. Student Accessibility and Welfare will contact the student to notify of the outcome of the review. If documentation is sufficient, the student will be asked to set up an appointment or receive approval via email.  If declined or if more verification is needed, the student will be notified and provided an opportunity to resubmit verification of religion.

STEP 4. If approved for accommodations, Student Accessibility and Welfare will provide a Reasonable Accommodation Notice to Instructors to the student. It is the student's responsibility to send the notice to their faculty for accommodation implementation.
