Please list all of the assessment tools you use when evaluating clients at this facility

The assessment tools we use at the VA are range of motion, Semmes Weinstein, test pinch and grip strength with a dynamometer, 9 hole peg test, VAMC SLUMS Examination, SCATBI, and The May-Portland.

Of the assessment tools utilized at this setting, name and describe one that you were not familiar with prior to this fieldwork? If there are none, please describe a challenging evaluation session you had with a client.

One of the assessments I was not familiar with prior to this internship was the VAMC SLUMS Examination.  It is very similar to the Mini Mental State Exam. The SLUMS exam assesses orientation, working memory, delayed recall, and auditory recall. We use this exam with our veterans diagnosed with PTSD/mTBI to assess their cognition. The max score is 30, a score of 27-30 indicates "normal" cognition, a score from 21-26 indicate "mild neurocognitive disorder" and a score of 1 to 20 indicates "dementia". We give this exam at their initial eval and at discharge to track their progression.
