  • Please list all of the assessment tools you use when evaluating clients at this facility
    • Cognitive assessments such as: Texas Functional Living Scale, MOCA, SLUMS, ACL, CPT.
    • Typical assessments such as: manual muscle testing, Orientation questions, BORG exertion scale.
  • Of the assessment tools utilized at this setting, name and describe one that you were not familiar with prior to this fieldwork? If there are none, please describe a challenging evaluation session you had with a client.
    • I had never seen the TFLS, which seems to be a pretty short yet effective assessment of four areas: time, money and calculation, communication and memory.  There is actual money that patients are asked to count and calculate change with, bill-paying and calendar tasks.  I feel this test is easy enough to give and offers good information regarding patient's ability to live independently or not and the help they may need.
