• This facility has occupational, physical, and speech therapy services. For patients who receive more than one type of therapy, the therapists often converse with one another in order to remain up to date on the patient’s progress in all therapies.

• This facility is an outpatient rehabilitation center. For adults, we mainly see orthopedics, CVA, and TBI.

- List three fieldwork objectives/ goals you and your fieldwork supervisor (FWS) have established. Choose one of those objectives and share how you believe this goal will be met.

• Demonstrate the ability to independently complete an OT evaluation.
  • I plan to meet this goal by first observing my FWS doing evaluations, then when comfortable with the evaluation process I will begin doing evaluations with my supervisor there for support and questions. After completing a few evaluations with minimal assistance, I will hopefully be able to complete evaluations without any assistance.
• Demonstrate the ability to appropriately interact with and refer to other disciplines.
• Demonstrate the ability to independently plan and complete a client session.
