Please discuss how you incorporate occupation-based interventions in this setting.

In our home-based therapy sessions with our pediatric clients, we incorporate occupation by working on the form of their handwriting and their pencil grasp, practice dressing and undressing, eat snacks in order to work on utensil use, and play age-appropriate games in order for optimal social participation.

Describe two issues that limit your ability to use occupation-based interventions and discuss strategies that could be used to minimize these barriers.

The most limiting factor for incorporating occupation based interventions into our treatment sessions is the child's behavior.  Many of our clients have ASD and sensory processing disorders, which causes them to require a lot of sensory/vestibular/proprio stimulation alternated with functional tasks.  Because of this issue, time limitations also make it difficult to fit many occupations into a treatment session. Usually, we spend time talking to the parents/caregivers and giving recommendations for home since we only see our clients 1-2 times per week, which takes up even more of our session time.  In order to overcome these barriers, I think it would be a good idea to incorporate sensory stimulation into occupation-based interventions, such as practicing handwriting using shaving cream or sand. It may also be necessary to set aside one treatment session per month that is solely based on parent/caregiver training and discussion in order to eliminate the time constraint that comes from spending too much time with the parents during each session.
