Please list the areas you and your FWS identified in your mid-term evaluation that you need to concentrate on for the next 5 weeks of this fieldwork rotation.

1. My FWS and I have decided that I need to collaborate with the veteran's family members and significant others more throughout through the evaluation/treatment sessions. 

2.  I need to establish goals/occupations with the veteran.

3. Work on time management, especially when fabricating orthotics for a veteran. 

4. Initiating contact/communication with colleagues about shared veterans. 

Discuss how you plan to meet these new objectives/ goals.

1. When a veteran's family member is present, I will include them in the session by asking them questions about the veteran.  Ask them if they have seen any differences, what their concerns are, and what deficiets they see. 

2. During the evaluation process, I will ask what the veteran his/her goals are, what occupation do they want to resume, and ask more questions about they're past/future.

3.  I will be mindful of the time when fabricating an orthotic. I will keep in mind the time it will tack for setting up, time for fabrication, and clean up.

4. My FWS and I are part of the mTBI team at the VA.  I will have open communication with other therapists and team members , especially when trying to decide if a veteran is malingering.
