Objectives – Organizational Chart

- Where does occupational therapy fit in the organizational chart for your facility?

I work at two different Brook Rehab facilities. On Mondays, I work at an outpatient clinic doing low vision therapy. Besides the Low Vision Optometrist, we are the main ones who assist the client. Tuesday-Friday, I work at Brooks Rehab Hospital. I am on a team with physical therapy, speech therapy, neuro psychology, medical doctors, and nurses. 

- Of the following categories, which best describes the type setting of this fieldwork experience?

    - Orthopedics, Acute, SNF, Pediatrics or Rehab (acute, outpt, etc)

Monday is low vision therapy at an outpatient clinic.

Tuesday-Friday is rehab.

List three fieldwork objectives/ goals you and your fieldwork supervisor (FWS) have established. Choose one of those objectives and share how you believe this goal will be met.

1. At 6 weeks, I will be in charge of evaluating and treating 100% of the caseload.

    Each week, I will take over one more patient until I am working with them all. I already have one person that I treat and document on.

2. I will be comfortable using different types of technology on the floor like the DynaVision and Bioness.

3. By 3 weeks, I will document an entire evaluation.
